I hereby grant Math Foundations, (DBA Trapezium Math Club) hereafter called “Math Foundations”, permission to publish information about me and/or my minor child (including, for example, name and likenesses obtained on or used on video or by photography, and/or description of events or characteristics) as provided by the developer Angela McIver (hereafter called the “Developer”) in a publication (and its future revisions, editions, translations, adaptations, and other formats including social media or web-based applications as may be devised) I hereby permit Math Foundations, its successors, assigns, affiliates and licensees (collectively, “Math Foundations”), to use the following for the purpose of advertising, trade or promotion in perpetuity throughout the world in connection with math club activities. Please note, we will never identify your child by last name.

I hereby grant to Math Foundations, (DBA Trapezium Math Club) hereafter called “Math Foundations”, ) the right and permission to use in perpetuity, my child's name, likeness, image, voice, recorded voice, appearance, biographical information, statements, performance and/or testimonial(s) (collectively, “Appearance”) in any manner and in any media, now known or later developed, throughout the world, at any time, for the purpose of advertising and publicizing Trapezium Math's products and services, without review, permission or compensation of any amount or kind whatsoever. Trapezium Math shall have complete ownership of any recording, product, copy, presentation or other material or file containing or featuring my child's Appearance (“Product”), including copyright interests, and I acknowledge I have no interest or ownership in the Product (or any portion thereof) or its copyright. This grant includes without limitation the right for Trapezium Math to edit, abridge, augment, title, or create a compilation from my Appearance in whole or part as Trapezium Math may elect in its sole discretion.
