My child is enrolled as a student in the Cheltenham School District as indicated below.

Please know that we take your child’s online privacy and security seriously. Please indicate that you have read our privacy policy by checking the box above.

Your child’s voice and image will be recorded as a part of their participation in Clubhouse/Clubhouse Jr. Additionally, your child’s name will be used during the session.

Our teachers use Zoom video conferencing to connect with your child as part of our group class. In addition, your child’s name will be used during the sessions.

Trapezium Math provides full-length video and audio recordings of group classes ONLY to Trapezium Math Staff. We do NOT provide recordings to other Trapezium Math parents nor to other Clubhouse/Clubhouse Jr. students. If Trapezium Math discovers a video or audio clip that is useful for demonstration purposes, Trapezium Math will ALWAYS seek signed parental consent for such use.

I give permission for my child to participate in Trapezium Math Club provided by the Cheltenham School district.

I understand that the Trapezium Math Club kit remains the property of the Cheltenham School District and that I am granted use ounder the following conditions:

I will return the Trapezium Math Club kit to Cheltenham School District when notified or at the end of the semester, in as good condition as when it was loaned, allowance being made for wear caused by careful use.

If the Trapezium Math Club kit is lost or damaged by carelessness or avoidable accident, I will replace it.

I may be held responsible for replacing a kit that the Cheltenham School District considers marked, torn or defaced.
